Paperwork & Forms
New patient forms allow us to review your dental history and prepare for your appointment. If you have dental insurance, we ask that you email or call our office with your insurance information ahead of time.
Completing forms in advance can help save time on the day of your visit.
If you’re unable to complete forms online, paperwork can also be completed at the clinic prior to your appointment.
Insurance & Payment
We’re happy to help answer questions about insurance coverage and payment options ahead of your visit.
If you plan to use dental insurance, please email previous records to info@heatherstreetdental.com with your name and the label "previous records". Coverage and eligibility vary by provider, and we’re always happy to review this with you.

Referrals & Previous Records
If you’ve been referred to Heather Street Dental or are transferring care from another clinic, any previous dental records or X-rays can be sent to us in advance via email.
This helps ensure continuity of care and avoids unnecessary duplication of imaging or treatment.
